Allergy Action Plan: Complete this plan with your student's doctor to provide individualized information about how to manage your student's life-threating allergy at school. To keep, or carry emergency allergy medication at school, this document must be completed and signed by the student's parent and doctor. Return the completed form to the front office of your student's school or your school nurse.
